When your business, organization or club becomes an official Community Supporter, you will be supporting the most highly acclaimed and accredited assistance dog organization in the United States. Just submit this commitment form below, and upon approval, Canine Companions will follow up with a link to our Give a Dog a Job Community Supporter Toolkit featuring turnkey marketing and PR materials plus a step-by-step guide for your in-store and online fundraiser.



This commitment between Canine Companions for Independence and your company outlines how you will help collect donations to benefit Canine Companions. Your company commits to transmit one hundred percent (100%) of all donations to Canine Companions through a quarterly check. If your company fails to do so, Canine Companions maintains the right to terminate this agreement.

Your company agrees to pay all costs and expenses associated with transmitting donations and conducting fundraising, including printing any fundraising materials. During the term, you may link your website to the Canine Companions Give a Dog a Job website at, provided that Canine Companions may withdraw approval for such link at any time. The license for all materials expires automatically at the end of the term.

This commitment may not be transferred to any other organization or amended without written agreement.

Please understand that Canine Companions for Independence cannot approve supporter relationships that are incompatible with our mission or fundamental principles. (Examples include companies that sell firearms, tobacco or adult materials).

Community Supporters and their customers and employees are positioned as donors to our mission, and this designation does not serve as an endorsement of products or services.
